This form is for organizations requesting sponsorship support from CBH CO-OP. Please provide details about your event or project, how the sponsorship will be used, and how CBH CO-OP will be recognized. All requests are reviewed on a regular basis, and you will be contacted once a decision has been made.

This form is intended for sponsorship requests—support for events, programs, or initiatives where CBH CO-OP will be recognized as a sponsor (e.g., logo placement, banners, public acknowledgment).
